The study of population health is focused on understanding health and disease in community, and on improving health and well-being through priority health approaches addressing the disparities in health status between social groups."
(Australian Institute of Health and Welfare, 2016)1

Accurate and up to date information and ongoing stakeholder engagement is critical to help identify and prioritise the health and access needs of our communities. The information provides an evidence base on which to make decisions about where to place and purchase services and resources. In conjunction with consumers and health professionals, we are then able to develop and plan cost effective solutions to improve health care outcomes across our region. Continual performance monitoring and evaluation of the services we commission will ensure they continue to address the needs of the population.
