Funded Primary Health Care services

Primary Health Care Services that were previously administered by Medicare Locals have now been commisioned for the 2016-17 period.

2016-17 funded services

HNECC PHN does not provide health care services, instead we fund health care providers to deliver a range of primary health care services that are appropriate and relevant to the needs of their communities.

Below are links to the list of all funded service providers, their locations and the contracted services.

Referrals to Health Service Providers – New England/North West

Referrals to Health Service Providers – Central Coast

Referrals to Health Service Providers – Hunter

The new contract period begins on July 1, 2016, and from this period the PHN will monitor the delivery of these services to ensure that the needs of patients and GPs are being met. The list of providers has details of clear contact points to help resolve any potential issues.  From August 2016 assessment and treatment waiting time targets for all mental health service providers will be made available monthly via this website.
