Healthy Weight Initiative Program offered free to practices

The PHN has developed an innovative program of Weight Management for Primary Care that targets patients at risk of developing chronic disease. The program uses a unique, online platform that has built in health screening, educational and lifestyle modification tools. It is currently being used in a research trial in more than 20 practices and achieving great results. We are offering the program software licence to an initial group of 100 practices free of charge for the first 12 months including initial training and ongoing support.
